STEP 1
New patients start with us by completing our preregistration form here online. This step ensures we have all the necessary information to provide you with the best possible care. The form includes basic personal details and insurance information.

Upon completion of preregistration, you will receive a confirmation message along with intake forms to complete. Please select an appointment time that aligns with your schedule. Our scheduling options are designed with flexibility to accomodate your lifestyle.

On the day of your appointment, simply show up at our office or log in to our secure telehealth platform for your online appointment. Our experienced healthcare professionals will be ready to assist you, ensuring you receive the care and attention you need.

During your first visit, you will undergo an initial assessment where we focus on the reason for your appointment, discuss your medical/psychiatric history, current symptoms, social circumstances, and treatment goals to develop a personalized treatment plan.

The frequency of follow-up visits is tailored to each individual based on the severity of your illness and the type of medications prescribed. Appointments can vary from monthly for controlled substances to every 3-4 months for stable medications.

In case of an emergency, please call 911 or go to the nearest emergency room. For urgent but non-life-threatening issues, contact our office or message your provider in your portal.

We will send your prescription if needed to your local pharmacy. Should a medication that we recommend be too expensive either you or the pharmacy can contact us with an affordable alternative or in some cases we may need to appeal to your insurance to cover the treatment.

We ask for at least 24 hours notice for cancellations. Missed appointments or late cancellations may incur a fee.
